You can access SimplySign, the LCS authorization e-sign feature, directly through the Client Portal.

We take the hassle out of finding the correct authorization by finding them for you. To save you time and money, state and facility-specific authorizations are automatically produced during the order entry process.  Generic authorization forms are available below to download. Also, you can conveniently find blank state and facility-specific authorizations on our Client Portal. Once logged in, click on Authorizations at the top of the homepage.
